During a colonoscopy, a long, flexible tube with a camera is inserted into the rectum, allowing the doctor to examine the entire colon. If polyps or suspicious lesions are found, they can be removed or biopsied during the same procedure.

For high-risk individuals, colonoscopies are typically recommended starting at age 40, 10 years before the youngest case in the immediate family, whichever comes first. Depending on the findings and the specific risk factors, repeat procedures may be advised every 3–5 years.

Flexible sigmoidoscopy is a shorter, less invasive version of a colonoscopy. It examines only the lower part of the colon (the sigmoid colon and rectum). While not as comprehensive as a complete colonoscopy, it can still detect cancerous or precancerous growths in the lower bowel.

For some high-risk individuals, especially those who cannot tolerate full sedation or bowel prep, sigmoidoscopy may serve as a viable alternative when combined with other tests like stool-based screening. However, because it misses lesions in the upper colon, it is not the preferred method for those with significant familial or genetic risk.

CT Colonography: A Non-Invasive Imaging Option

Also known as virtual colonoscopy, CT colonography uses low-dose radiation and computer-generated images to create a detailed view of the colon and rectum. While it does not require sedation, it does involve bowel prep and air insufflation into the colon.

CT colonography is often used when traditional colonoscopy is not feasible due to medical conditions or patient preference. It can detect polyps larger than 6mm and reveal abnormalities outside the colon, which traditional colonoscopy cannot.

However, one key limitation is that polyps cannot be removed during the scan. A traditional colonoscopy will still be required for removal and biopsy if abnormalities are found. For high-risk individuals, this added step makes CT colonography a secondary option rather than a first choice.

Stool-Based Screening: Supplemental Tools for High-Risk Populations

Stool-based tests detect hidden blood or abnormal DNA in the stool and are less invasive than endoscopic procedures. Common types include:

Fecal immunochemical test (FIT) – detects hidden blood in the stool

Guaiac-based fecal occult blood test (gFOBT) – an older method for detecting blood

Stool DNA test (e.g., Cologuard) – identifies DNA mutations and blood associated with cancer

These tests are typically used for average-risk populations, but they can sometimes complement colonoscopies in high-risk individuals, especially if the latter is delayed or unavailable.

However, stool-based tests should never replace colonoscopy for those with high-risk factors, as they lack the sensitivity to detect polyps and early cancers reliably in this population.

Genetic Testing and Surveillance for Hereditary Syndromes

For individuals with known hereditary syndromes like Lynch syndrome or FAP, genetic counseling and testing are crucial. These conditions dramatically increase the lifetime risk of developing colon cancer and often at a much younger age.

In such cases, colonoscopy screening may begin as early as age 20, with intervals as frequent as every 1–2 years. Proactive surgical options, such as colectomy, may even be discussed for those with exceptionally high risk.

Emerging blood-based biomarker tests and liquid biopsies are being researched as potential tools for ongoing surveillance in genetically predisposed individuals. While not mainstream, they may soon offer additional safety nets for these vulnerable patients.

Combining Screening with Lifestyle Modifications

For high-risk individuals, screening alone is not enough. Lifestyle changes can significantly reduce the likelihood of cancer developing or recurring. These include:

Maintaining a healthy weight

Eating a high-fiber, low-processed diet

Limiting red and processed meats

Avoiding tobacco and excessive alcohol

Exercising regularly

These changes are especially critical for those with inflammatory bowel disease, as chronic inflammation contributes to cancer development. Medical therapies to control inflammation and regularly monitor disease activity can also reduce long-term risk.
